Octave Harmonics

When I do this it's usually 1st finger and 3rd finger and it's all a bit slow. Looking at this Tommy Emmanuel video it looks like he is using several digits, can anyone do this and how do you achieve that fluidity?

I think I've got the mechanics of this now. He is using his index and thumb for the harmonic and his third finger for the alternate string. Across the open strings it would be E D A G D B G E and back. 
Just tried it and it's pretty hard on the left hand when fretting chords across the 6 strings.
