3 pup, 5 way switch wiring problem

Just wired 3 pups to a 5-way switch, and it all 'looks' OK as far as I can tell. All is not well though - The middle pup is on in all positions? Is there any logical explanation of what I might have done?

    Is the switch fitted through a thicker than usual pickguard (or through wood) and/or the slot is not quite long enough, so it isn't fully clicking into the end positions?

    The switch is operating fine in terms of clicking into the right place.

    I think I'll just have to go over everything again very carefully when I get back home. It's just a test at the moment, using two SCs instead of a middle pup, so I wasn't quite as meticulous as I usually am.
