where can I get a input plate like this?

thinking of creating my own version of this cab with a zilla 2x12 I have lying around.

can't find these input plates (3 inputs, switchable to 1 input for both speakers at 16 ohms) or 2 separate 8 ohm speaker inputs:

You don't need to - you can do what you want with just two jacks and a switch, or even with two jacks using their own switching as long as both inputs can share a common ground (eg from a stereo amp).
