1) Poor ground connection.
2) One coil out of circuit.
3) The four output conductor wires connected in the wrong order.
Q1: Have you rewired the guitar recently?
Q2: Are all of the control components original?
Some of those parts are now thirty five years old and were not necessarily the highest quality to begin with. I would check the on/off switch first.
