Without watching the video, I going to guess that it's the spike under the paper cup trick.

I've seen this one go wrong before, and when it does it's always an audience member that gets hurt.

There is a newer version with a ball-shuriken that people have tried to perform before they are ready. That's really nasty.

I've burnt myself a few times but I've never impaled anything so far!
