One problem is that many strap button screws are too small - both too narrow and too short - to really grip the wood properly. I very often replace them with larger ones - it's surprising how much larger a screw you can get through most standard strap buttons, compare to the supplied ones.

But then I probably see a disproportionate number of the guitars where the screws pull out, being a repairer. It is often still quite worrying how small a screw many manufacturers think is good enough, given what it has to hold.
